Dear Group administrators,

The benefit activation date for those who registered up to 04/01/2022 is around the corner (06/29/2022). Please take the time to verify that the following information is on file for each member of your group and is accurate:

• Full name as in a government issued document
• Day and month of birth. we do not need the year of birth
• At least one beneficiary name provided
• Primary residence is USA or Canada and the information is provided.

Also reach out to your members to make sure they are not enrolled with SOBATAM more than once. Please  note that a member who enrolls  more than once,  will only receive the benefit for one enrollment. The benefit may be denied altogether if  it is established  that the multiple enrollment was intentional.
